Striving for your progress in the community

Community Support Habilitation services provide intensive case management to help individuals who are considered to be “at-risk” within the community foster independence, integration, individuality, and productivity.

Our program includes the provision of family involvement, which enhances flexibility and freedom of choice. We provide a personalized care plan to help individuals participate in adaptive skill development, assistance with activities of daily living (hands-on), community inclusion, and relationship building and training. This program also offers support for independence in travel, transportation, adult educational, development of social skills, leisure skills, self-advocacy, informed choice skills and appropriate behavior development to help the individual live independently within their community.

This program offers:

  • Adult Companion includes non-medical care, supervision, socialization, and daily living skills provided to an adult.
  • Individualized Home Support for the acquisition, improvement, and retention of skills. This service provides necessary assistance to achieve personal habilitative outcomes in order to enhance individuals’ abilities to live in their community, as specified in the Individual Plan of Care.
  • Personal Supports for the assistance necessary to meet day-to-day activity, daily living needs, and reasonably assures adequate help at home and in the community to carry out personal outcomes.
  • Senior Supports for the assistance of daily activities that our elderly population requires. Our goal is to provide safety and independence in their own home or to offer support for overtaxed family caregivers.
