Community Support Habilitation services provide intensive case management to help individuals who are considered to be “at-risk” within the community foster independence, integration, individuality, and productivity.
Our program includes the provision of family involvement, which enhances flexibility and freedom of choice. We provide a personalized care plan to help individuals participate in adaptive skill development, assistance with activities of daily living (hands-on), community inclusion, and relationship building and training. This program also offers support for independence in travel, transportation, adult educational, development of social skills, leisure skills, self-advocacy, informed choice skills and appropriate behavior development to help the individual live independently within their community.
This program offers:
- Adult Companion includes non-medical care, supervision, socialization, and daily living skills provided to an adult.
- Individualized Home Support for the acquisition, improvement, and retention of skills. This service provides necessary assistance to achieve personal habilitative outcomes in order to enhance individuals’ abilities to live in their community, as specified in the Individual Plan of Care.
- Personal Supports for the assistance necessary to meet day-to-day activity, daily living needs, and reasonably assures adequate help at home and in the community to carry out personal outcomes.
- Senior Supports for the assistance of daily activities that our elderly population requires. Our goal is to provide safety and independence in their own home or to offer support for overtaxed family caregivers.
After completing an intake assessment, we will create a plan to reduce family concerns and ease the burdens of family caregiving.
Individuals participate in a broad range of structured group activities which are community-based and Virtual activities via telehealth.

EPRINE COMMUNITY SERVICES offers day activities to help individuals integrate directly into the community. This highly individualized program is designed to maximize their employment readiness, social interactions, arts, performance with daily living skills, and independent living skills in the community.

Community Enrichment Program
The Community Enrichment Program is a community-based Day Habilitation “Without Walls” program that serves motivated young adults who wish to be active and productive. We assist adults with intellectual disabilities who require life skills training, which is beneficial in their career exploration. The program promotes their independence and provides opportunities to grow and mature into responsible adults.
This model significantly increases the community-based experiences and job readiness skills of new graduates. The program prepares individuals by utilizing resources available in the community to integrate them as they adjust from school to adult life.
What are the goals of Eprine’s Community Enrichment Program?
Life-Appropriate Activities: Implemented and reflective of those things of interest to and appropriate for adults rather than activities designed and geared for children.
Offer Variety & Choice Activities: Opportunities are varied to develop and practice skills and that can be applied to new settings. Offering a variety of activities assists with behavior reduction, engagement, decreases boredom and provides individual with the opportunity to identify strengths, abilities, preferences, support needs, etc.
Emphasize Community Experiences: Eprine Day Services moves away from traditional programming and emphasizes real community experiences that enables program participants to grow as an individual, gain respect for others, increase civic participation and direct their own lives.
Individual Day Services
Individual Day Services provides individually tailored supports that help consumers gain or maintain skills to participate more fully and productively in work, leisure or community activities. This may include the development of independent functioning skills, relationship skills, exploring job interests or retirement programs. The services are individualized to meet the needs of the participant. These services are person centered, valued based and goal oriented.

Supported Employment
Understanding the importance of work and the growing number of individuals with special needs who want to achieve employment, Eprine offers a full continuum of opportunities. Eprine supported employment program help people to enhance their social skills and work ethics as they prepare for work. Opportunities including job training, work experience, job placement and post-placement support services. An important part of the vocational continuum is our Career Group, groups that provide an opportunity for those interested in work to benefit from the experience of those already working. Eprine services places qualified candidates with employers. Job candidates who go through our program are extremely reliable and are always pre-screened to make sure they will be a good fit for the company looking to train or hire and its unique work environment.
EPRINE COMMUNITY SERVICES offer mental health services to treat children, adolescents, and adults who are facing challenges in behavioral, emotional, anxiety disorders, and other psychological conditions.
